Fireplace Service Plan Membership

This Rewards Annual Membership with Fireplaces Unlimited will be good for one-year terms unless you choose to cancel prior to the renewal date or your Rewards Membership is canceled, terminated, or discontinued by you or Fireplaces Unlimited. Your payment method will be charged based on the current rates in effect at the time of the renewal. Rates and membership details are subject to change. This membership has no monetary value or reimbursement if benefits are not used. You are required to notify Fireplaces Unlimited immediately of any change in your name, billing address, service address, email address, telephone number, credit card, bank, or other account information. Your Rewards Annual Membership entitles you to a wide range of attractive benefits, including priority service and discounts. Eligibility for your Rewards discounts is contingent on your Rewards Membership being in good standing for at least six months. The first visit will be charged at full price, and the discounts apply to any visits after the first visit. Included with your membership is a Fireplace Tune-up and Professional Cleaning annually (one before summer or one before winter). We will attempt to contact you via email, mail, or telephone/text to schedule your included tune-up, or you can contact us directly to schedule your appointment. If you fail to take advantage of these benefits during your annual term, these annual benefits are lost for the applicable Annual period and are of no monetary value. Each Tune-up must be performed before the end of each season (before summer or before winter). If you fail to take advantage of these benefits, these fireplace Tune-Ups and Professional Cleanings become void, have no monetary value, and are not a contractual component of this agreement. If the location of the home is not in Lubbock County, a mileage fee may be assessed. By signing the agreement, you agree to a minimum of 12 months of membership.
